The type of personal information we collect
We currently collect and process the following information:
- Personal identifiers, contacts and characteristics for example, name, address,
contact details, health conditions, programs, and recommendations.
How we get the personal information and why we have it
All of the personal information we process is provided to us directly by you for one of the following reasons:
- Your initial contact
- Referral into the service
- Initial assessment
- Specific assessments
We may also receive personal information indirectly, from the following sources:
- Your GP
- The courts
- Your service provider
We use the information that you have given us in order to inform us of your needs and helps us to identify the best way to support and help you.
We may with your permission share this information with other third parties which are relevant to you and your care; such as the Court or GPs.
We will need to process your personal data, and in particular personal data relating to your health, for the following reasons:
- To facilitate in the fulfilment of the above services. This can include, but is not limited to, sourcing relevant medical records, arranging an appointment.
- To process your personal data in line with any legal obligations we are required to comply with. This will include our obligations to you in relation to the processing of your personal data.
We can assure you that we will only ever process the minimum personal data required to carry out the services identified above.
Your personal data will not be transferred to or shared outside of the United Kingdom.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
- Your consent. You are able to remove your consent at any time. You can do this by contacting All-in-One Occupational Therapy, in writing with your request.
- We have a contractual obligation.
- No confidential information will be passed or accessed by a third party.
How we store your personal information
- Your information is securely stored, encrypted and saved on a password secured external drive.
We keep personal information for seven years. We will then dispose your information by shredding/deleting all identifiable information.
We will only use your personal data to the extent necessary to comply with our legal obligations (for example for HMRC financial accounting obligations).
Your Rights:
Under the Data Protection Act, you have the following rights in relation to your personal data.
Right to Access - You have a right to request a copy of your personal data.
Right to Rectification - You have the right to ask us at any time to make any corrections or remove any personal data that you believe is inaccurate.
Right to Erasure - You have the right to request the erasure of the personal data. However, we may still need to retain personal data to comply with our legal obligations. In the event of a request, we will provide you with full details of the personal data that we are required to retain.
Right to Restriction - You have the right to request a restriction in the processing of your personal data.
Right to Objection – You have the right to object to the processing of your personal data. We will aim to respond to any requests relating to your rights without undue delay and in any case within 1 calendar month of receipt of your written request. If we are unable to comply with a request for any reason, then we will provide you with a full justification in writing within 1 calendar month of receipt of your request.
